An integral part of “person-centered” care includes providing a social worker to assist our residents and their families. The social worker provides advocacy, a link to support and resources, and reassurance that a positive living experience is a shared goal as a loved one’s transition evolves.
Social Services Encompasses:
- Acting as a liaison between resident/family and community resources. (Examples: vision/hearing loss programs, psychiatric services and other referrals.)
- Advocating for resident/family to address concerns or changes.
- Serving as a communications resource for hospitals, rehabilitation facilities, and nursing homes.
- Counseling on problem solving complaints or concerns, promoting interaction and helping residents feel part of our community.
- An integral part of the care and service team is to identify resident’s needs and work with families to be part of the overall care and service to the resident.
- Attending and contributing to off-site care conferences to improve transition to memory care at move-in and throughout the resident’s living experience at The Alton.
- Serving as point person for initial transition to environment, including assisting with the logistics of moving, offering supportive services during initial move-in and providing feedback to families.
- If another level of care is needed, as noted by the care team (including the resident staff and family), our social worker will work with everyone to help find the best possible setting to meet those care needs.
